Can I transform my body back to how it used to look?

Yes it is possible. It will take discipline, aerobic exercise, free weights and an exercise ball. First, start drinking lots of water, no sodas, eat a low calorie balanced diet and vitamins. Second, it helps if you enjoy the aerobic exercise (bicycle, running, walking, swimming) because you'll stay with it so choose things you like to do and vary your workouts. Believe in yourself, visualize the changes and stay positive!
